What Is Losartan?
Losartan belongs to a class of blood pressure drugs called angiotensin receptor blockers (ARBs). Angiotensin II is a hormone that causes blood vessels to constrict (narrow). Losartan blocks the receptor that angiotensin II binds to — so blood vessels stay relaxed and dilated, reducing blood pressure.
Brand name: Cozaar. Generic losartan potassium has been available since 2010 and has largely replaced the brand. It is on many pharmacy $4–$10 generic lists.
What Does Losartan Treat?
| Indication | Notes |
|---|---|
| Hypertension (high blood pressure) | First-line treatment; reduces heart attack and stroke risk |
| Diabetic nephropathy | Slows kidney disease progression in type 2 diabetics with high BP and protein in urine |
| Heart failure | Used in patients intolerant to ACE inhibitors |
| Marfan syndrome | Off-label; reduces aortic root dilation rate |
| Stroke prevention | Reduces stroke risk in patients with hypertension and left ventricular hypertrophy |
Dosage
- Starting dose: 50 mg once daily
- Range: 25–100 mg daily (in one or two doses)
- Maximum: 100 mg/day
- Can be taken with or without food
- Effect on blood pressure begins in about 1 week; maximum effect at 3–6 weeks
🚨 Do NOT take during pregnancy
Losartan (and all ARBs and ACE inhibitors) can cause serious fetal injury or death when used in the 2nd or 3rd trimester of pregnancy. Stop losartan immediately and contact your provider if you become pregnant while taking it. A different blood pressure medication safe for pregnancy will be prescribed.
Side Effects & Warnings
Losartan is generally very well tolerated. Most people experience few to no side effects at standard doses.
- Dizziness or lightheadedness — especially when standing (orthostatic hypotension)
- Elevated potassium (hyperkalemia) — important in patients with kidney disease or those taking potassium supplements or certain diuretics
- Mild fatigue
- Upper respiratory symptoms (cold-like) — less common than with ACE inhibitors
📌 No ACE inhibitor cough
One of the biggest advantages of ARBs like losartan over ACE inhibitors (lisinopril, enalapril) is the absence of the chronic dry cough that affects 15–20% of ACE inhibitor users. If you developed an ACE inhibitor cough, your provider may have switched you to losartan for exactly this reason.
Frequently Asked Questions
How long does losartan take to lower blood pressure?
Losartan begins lowering blood pressure within a few hours of the first dose. Significant clinical effects are usually seen within 1 week, and the full antihypertensive effect takes 3–6 weeks. Blood pressure will typically rise again within 24 hours if you miss a dose, which is why consistent daily dosing is important.
Can I stop losartan if my blood pressure is normal?
Do not stop losartan without talking to your doctor. If your blood pressure is now normal, it is likely because losartan is working — not that the underlying hypertension has resolved. Stopping suddenly can cause blood pressure to rebound to dangerous levels. Your provider may eventually taper the dose or switch medications, but this should be done under supervision.
Should I avoid potassium on losartan?
ARBs can raise blood potassium levels (hyperkalemia). While most healthy patients do not need to restrict dietary potassium, you should avoid potassium supplements and potassium-sparing diuretics (like spironolactone) unless specifically prescribed with losartan. Patients with kidney disease are at higher risk and should have potassium levels monitored regularly.
Is losartan the same as Cozaar?
Yes. Cozaar is the original brand name. Generic losartan potassium is medically identical and has been available since 2010. Most pharmacies dispense generic losartan, which costs dramatically less than the brand.
How much does losartan cost without insurance?
Generic losartan potassium is extremely affordable — typically $4–$15 per month with a free Rx.com discount card at major pharmacies. It is on many pharmacy $4 generic lists. Compare prices near you at /drug/losartan-potassium.
