It is FDA-approved for the acute treatment of migraine attacks, with or without aura, in adults, and in adolescents ages 12 to 17 with a history of migraine headaches usually lasting 4 or more hours (when untreated). It is meant to stop a migraine that has already started, not to prevent migraines from occurring, and it is not indicated for hemiplegic or basilar migraine.
The usual adult dose is 6.25 mg to 12.5 mg taken at the onset of a migraine; a second dose may be taken after at least 2 hours if symptoms return, up to a maximum of 25 mg in 24 hours. The original brand-name product, Axert, has been discontinued in the U.S., so almotriptan malate is currently available only as a generic.

Safety information for almotriptan malate
Almotriptan malate is not appropriate for everyone. It should not be used by people with ischemic or vasospastic coronary artery disease (including Prinzmetal's angina), a history of stroke or TIA, hemiplegic or basilar migraine, peripheral vascular disease, or uncontrolled high blood pressure. It should not be combined with another 5-HT1 agonist or an ergotamine-type medication within 24 hours, and combining it with SSRIs, SNRIs, or other serotonergic drugs raises the risk of serotonin syndrome. Unlike some other triptans, almotriptan's FDA label does not carry an MAOI-specific restriction — a drug-interaction study with the MAO-A inhibitor moclobemide showed a modest increase in almotriptan levels with no dose adjustment needed — but you should still tell your doctor about all medications you take, including MAOIs. People with liver impairment or severe kidney impairment are not excluded from taking almotriptan, but the label calls for a lower starting dose (6.25 mg) and a lower maximum daily dose (12.5 mg in 24 hours) because clearance is reduced.
Even in people without known heart disease, triptans like almotriptan can rarely cause coronary artery vasospasm, chest or throat tightness, or, rarely, a heart attack or abnormal heart rhythm. Using this medication too frequently can also lead to medication overuse headache.
