What is Dofetilide ?

Dofetilide is a class III anti-arrhythmic medication primarily used to maintain sinus rhythm in patients with atrial fibrillation or flutter. It works by selectively blocking the rapid component of the delayed rectifier potassium current (I_Kr), prolonging the action potential duration and refractory period in cardiac tissue. Due to its potential to cause serious ventricular arrhythmias including Torsades de Pointes, it is initiated in a hospital setting with careful monitoring of the QT interval.
