What is Fintepla and how does it work?

Fintepla's active ingredient is fenfluramine, a serotonin-releasing agent (a 5-HT2 receptor agonist). Fenfluramine is the same drug substance that was previously sold for weight loss — including as part of the "fen-phen" combination (fenfluramine-phentermine) — and it was withdrawn from the U.S. market in 1997 after it was linked to valvular heart disease and pulmonary hypertension at the higher doses used for weight loss. Fintepla uses much lower doses of that same substance for a different purpose: as an add-on treatment to help control seizures, not for weight loss. Because Fintepla contains the same drug substance tied to those earlier heart risks, it carries a boxed warning and requires regular echocardiogram monitoring throughout treatment (see Safety information below).

The FDA has approved Fintepla oral solution as an add-on (adjunctive) therapy for seizures associated with Dravet syndrome and Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older. It's meant to be used together with a patient's other anti-seizure medications, not typically as a stand-alone treatment.
