Comúnmente comercializada bajo el nombre de marca Lyrica, también se utiliza en el tratamiento del trastorno de ansiedad generalizada. What Is Pregabalin (Lyrica)?
Pregabalin (Lyrica) is a Schedule V controlled substance and anticonvulsant with established efficacy in nerve pain conditions, fibromyalgia, epilepsy, and generalized anxiety disorder. It is structurally related to gabapentin but has more predictable absorption and is generally considered more potent per milligram. Pregabalin was the first drug FDA-approved specifically for fibromyalgia.
What Does Pregabalin Treat?
- Diabetic peripheral neuropathy — burning, shooting pain in feet and legs
- Post-herpetic neuralgia (PHN) — ongoing nerve pain after a shingles outbreak
- Fibromyalgia — widespread musculoskeletal pain and tenderness
- Spinal cord injury neuropathic pain
- Partial-onset seizures — adjunctive therapy in adults
- Generalized anxiety disorder (GAD) — widely used off-label for anxiety in the US
How Does Pregabalin Work?
Pregabalin binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system. This reduces calcium influx in hyperexcited neurons, slowing the excessive release of excitatory neurotransmitters like glutamate, norepinephrine, and substance P. The result is reduced transmission of pain signals and seizure activity.
Dosage Overview
- Diabetic neuropathy: 50 mg three times daily, titrated to 100 mg TID (max 300 mg/day)
- PHN: 75 mg twice daily, titrated to 150–300 mg twice daily
- Fibromyalgia: 75 mg twice daily, up to 225 mg twice daily (max 450 mg/day)
- Partial seizures: 75 mg twice daily to 200 mg three times daily (max 600 mg/day)
Pregabalin should be tapered slowly when discontinuing. Kidney function affects dosing — adjustments needed in renal impairment.
Side Effects
Common side effects include dizziness, somnolence (drowsiness), dry mouth, blurred vision, and weight gain. Pregabalin is a Schedule V substance due to potential for misuse in some patients.
