Hydralazine hydrochloride is a direct-acting arteriolar vasodilator. It works by relaxing the smooth muscle in the walls of small arteries (arterioles), which widens those blood vessels and lowers the resistance the heart has to pump against, in turn lowering blood pressure.

The oral tablet is FDA-approved for essential (chronic) hypertension, while an injectable (IV/IM) form is FDA-approved for hypertensive emergencies in a hospital setting. Hydralazine is also used off-label, often in combination with isosorbide dinitrate, as part of guideline-directed therapy for heart failure with reduced ejection fraction, particularly in Black patients or in patients who cannot tolerate ACE inhibitors or ARBs (this combination is sold under the brand name BiDil). It's also sometimes used off-label to manage high blood pressure during pregnancy, including preeclampsia.

Hydralazine works differently than most first-line blood pressure medications. Drugs like lisinopril (an ACE inhibitor), losartan (an ARB), and amlodipine (a calcium channel blocker) are typically tried first for essential hypertension, with hydralazine reserved for patients who need an additional or alternative option.

Because hydralazine can trigger a reflexive increase in heart rate, it's often prescribed together with a beta-blocker such as metoprolol to offset that effect. In heart failure regimens, it may be combined with isosorbide dinitrate and a diuretic like furosemide as part of a broader treatment plan.

Hydralazine HCl Safety Information

Hydralazine HCl does not carry an FDA boxed warning. The most notable risk with long-term or higher-dose use is a drug-induced lupus-like syndrome, joint pain, fever, rash, and a positive ANA blood test, which is more common in people who metabolize the drug slowly ("slow acetylators") and typically resolves after the medication is stopped. Some people also develop peripheral neuropathy (numbness or tingling), which may improve with vitamin B6 (pyridoxine).

Because hydralazine widens blood vessels, it can cause reflex tachycardia, low blood pressure, and, in people with coronary artery disease, angina or reduced blood flow to the heart, which is one reason it's often paired with a beta-blocker. It should be used with caution in significant kidney impairment, and some labeling lists coronary artery disease and mitral valve rheumatic heart disease as contraindications, along with known hypersensitivity to hydralazine.
