Drugs like lisinopril (an ACE inhibitor), losartan (an ARB), and amlodipine (a calcium channel blocker) are typically tried first for essential hypertension, with hydralazine reserved for patients who need an additional or alternative option.

Because hydralazine can trigger a reflexive increase in heart rate, it's often prescribed together with a beta-blocker such as metoprolol to offset that effect. In heart failure regimens, it may be combined with isosorbide dinitrate and a diuretic like furosemide as part of a broader treatment plan.
