What Is Prilosec and How Does It Work?

Prilosec is the brand name for omeprazole, a proton pump inhibitor (PPI) that works by irreversibly blocking the stomach's H+/K+ ATPase, often called the "proton pump," to reduce acid production. Prescription Prilosec is approved to treat GERD and erosive esophagitis, Zollinger-Ellison syndrome and other conditions that cause excess stomach acid, and duodenal or gastric ulcers, including as part of a combination regimen with antibiotics such as clarithromycin and amoxicillin to help eradicate H. pylori infection and reduce ulcer recurrence.

Prilosec OTC, the nonprescription version, is approved only for frequent heartburn occurring two or more days a week in adults, taken as a 14-day treatment course. Omeprazole does not relieve heartburn immediately — it can take one to four days of daily use for the full acid-reducing effect to kick in.

Its label does include several warnings and precautions worth knowing about, especially with long-term or high-dose use: hypersensitivity reactions, acute kidney inflammation (acute tubulointerstitial nephritis) that can occur at any point during treatment, Clostridioides difficile-associated diarrhea, low magnesium levels (which can lead to low calcium or potassium), vitamin B12 deficiency, an increase in stomach (gastric fundic gland) polyps, and a higher risk of hip, wrist, or spine fractures. Omeprazole can also interact with warfarin, reduce the effectiveness of clopidogrel, and interact with other drugs processed by the CYP2C19 enzyme.

The Prilosec OTC label limits use to 14-day courses, up to three times a year, and warns against taking it for longer than 14 days without talking to a doctor first.
